I had an image named "trinity.JPG" in my data directory. I tried adding a
new file named trinity.jpg and was prompted with: "rename the existing
version of "trinity.jpg". (the prompt gave the wrong case on the suffix). I
said yes, but the file name remained "trinity.JPG" in the data directory.

Welcome to the wonderful world of case insensitive, case preserving file
systems!

It's an operating system issue but I've added a workaround for release 0153.
